Little bit about ENDOSCOPY

These  information  gathered  by myself  & few of  my colleges  during  introductory period of our clinical (Ragama medical faculty - Sri Lanka) . I hope this will help you to gain some knowledge on  endoscopy…………

Different scopes & their indications

  •  Upper GI endoscope (1m)- Can visualize oesopghagus, Stomach & the 1st & 2nd parts of duodenum.(Oesophago-gastro-duodenoscopy)
  •  Colonoscope (160cm)- Used to visualize the whole colon upto caecum.
  • Sigmoidoscope- Used to visualize the sigmoid colon & descending colon upto splenic flexure. (the same instrument is used as in colonoscopy)
  • Cystoscope- Used to visualize the bladder & urethra. (Urethro-cystoscopy)
  • Bronchoscope- Used to visualize the respiratory tract.

Decontamination of scopes

  • Prior to sterilization instruments are cleaned manually.
  • Sterilization- Two solutions are found in the endoscopy unit.
  1. Cidex - Has side effects like sore throat & irritation of the eye.
  2. Perasafe - Mostly used because of less side effects.

Perasafe Advantages

  • Aldehyde free
  • Sporicidal
  • Tuberculocidal
  • Virucidal
  • Bactericidal
  • Effect lasts for 24 hours
  • Can be used only with stainless steel or inert material
  • The scope is bathed for 10 minutes in the disinfectant trolly containing the solution, before using it.

Preparation of patients for the endoscopy

In upper GI

Patient is kept fasted for 6 hours .

In colonoscopy

Asked patient to take light & low fiber diet for 3 day before admission.
then admit the patient4 klean preparations(sachet) given .they are given one by one, once in 2 hours.
After one sachet is given, the patient is asked to drink water & clear fluids.
(This loosens the bowel contents & washes out the bowels)

In sigmoidoscopy

Patient is kept fasted for 6 hours

give a klean enema next day morning . asked patient hold it for  45 min.

10% Xylocaine spray is applied to the patient’s throat in upper GI

Xylocaine has a topical anaesthetic effect, thus it anaesthetizes the throat.
( As gag reflex will disappear, the patient is thoroughly advised not to eat or drink for a period of 20 mins after the endoscopy to prevent inahalation into the respiratory tract.)

During the procedure, sedations are given to the patient to reduce the sense of pain.
  • Pethidine
  • Midazolam
Patient is given an antibiotic.
